Valley Care Community Consortium is co-hosting a community forum with Los Angeles County's Center for Health Equity on Wednesday, October 3, 2018 from 2:00 - 4:30 pm at the Zev Yaroslavsky Family Support Center in Van Nuys located at 7555 Van Nuys Blvd., Van Nuys, CA 91405.
